Accountable Care and Population Health Guide Our Future
With the current healthcare system rapidly becoming unsustainable, Summa Health is transforming the way we deliver care. With a growing accountable care organization now securely in place, our focus has shifted to caring for the overall health of the population we serve.
Our ACO — known as NewHealth Collaborative — features care managers, embedded in physician practices, working directly with our patients and their caregivers. A large network of clinically integrated primary care providers, as well as a robust group of aligned and affiliated physician specialists, are helping us achieve our "Triple Aim" objectives:
- Improving the experience of care for individuals
- Improving the health of populations we serve
- Lowering the total cost of care
And by achieving our objectives, we are developing a new type of patient experience — one where a team of clinicians not only work collaboratively in the treatment of the patient, but also in concert with the patient to make real lifestyle changes and to implement preventative healthcare measures as necessary.
In addition, we will also soon have one unified medical staff sharing a fully integrated electronic medical records system that will assist in the coordination of patient care and the sharing of patient information. Standardized care and "best practices" will be utilized throughout the organization and patient-center medical homes will become more and more commonplace.
